myHealthway is a self-monitoring service for patients with a chronic disease. The service combines the actions of collecting, transferring safely, storing and monitoring data in one application.

The app is intended to be used in cooperation with the care unit thus providing an interaction between the patient and his/her care unit for improved monitoring and communication in the care process.

For medical professionals myHealthway offers a new way of creating an interactive patient relationship, supported by modern database and smartphone applications. The myHealtway service is based on the Nokia Wellmate diabetes service developed by Finland’s leading diabetes doctors and experts. myHealthway Ltd. has developed the service further to cover also other chronic diseases like metabolic syndrome and hyper tension. The service allows medical staff to monitor the patient closely and to stay up-to-date concerning any circumstances affecting the patient’s situation.

For healthcare organizations, myHealthway offers an efficient way of organizing the monitoring of the treatment of patients with a chronic disease. It also makes it easier to incorporate new methods and to overcome future challenges. The application can be optimized for each patient separately
